DEPARTMENT: Outpatient and ID&A Residential Program
POSITION: Community Registered Nurse
STATUS: Full Time; 37.5 hours / week
SCHEDULE: 9:00 AM to 5:00 PM; Monday through Friday
LOCATION: Wilkinsburg & ID&A Residential Sites

Milestone Centers has earned certification from the Sanctuary Institute for providing trauma-informed care.
Responsibilities:
Provides nursing care under the direction of Milestone’s Director of Nursing and providers. Assists and provides necessary medical support and services to the Behavioral Health Outpatient program. Assists all staff and people served in the Intellectual Developmental & Autism (ID&A) program.
- Demonstrates and reflects a commitment to the mission and values of Milestone by serving others with respect, dignity, and compassion.
- Establishes effective working relationships with clients, families, providers, pharmacies and all members of the treatment team.
- Completes verbal orders from physicians and Certified Registered Nursing Practitioner (CRNP).
- Assumes responsibility for medication prior authorization with insurance companies.
- Monitors lab and test results including bloodwork for the Clozapine REMS program.
- Assumes responsibilities for maintaining medication samples and storage of Patient Assistance Program (PAP) medications.
- Provides Prescription Drug Monitoring Program (PDMP) reviews for Medical Doctor (MD) and CRNP as required.
- Collaboration in ongoing treatment planning through treatment team meetings and clinical supervision.
- Meets all paperwork/computer work deadlines.
- Participates in program evaluation and data collection.
- Provides coverage for Registered Nurse (RN) in outpatient and residential ID&A program as needed.
- Triaging phone calls based on urgency of request.
- Assist with ordering and inventory of medical supplies.
- Works with and assists the ID&A residential program team through:
- Focus on Everyday Lives and compliance with Title 55 Chapter 6400, 2380 and 6500 Regulations
- Observation, evaluation and providing prompt appropriate medical intervention as needed
- Participation in Discharge planning meetings for Residential and Life Sharing clients who have been hospitalized and ensure that discharge instructions are communicated to staff and implemented.
- Prepare and attend client appointments as necessary to ensure continuity of care as well as supporting staff in medical conversations
- Train staff and Lifesharing Providers on updated medical protocols and any additional medical training as needed
- Review of clients’ medical records, including but not limited to the Individual Support Plan (ISP), Annual Physical, Health Care Contact records and Lab reports and coordination and provision of health care services in all programs as needed.
- Develop, complete and implement Health Risk Screening Tool planning and all Lifetime Medical history documentation
- Communication of health information to medical personnel, as needed, and facilitating, supporting and coaching ID&A Supervisors and Staff as to how they should communicate with medical personnel
- Assisting in identification and development of resources for clients and families that support health and wellness and provision of these programs in a format which can be shared with clients and families
- Assisting in the provision of support and education/training to all ID&A Supervisors and staff related to the program clients’ medical diagnoses; prescribed medications; dietary guidelines and restrictions; mobility limitations and/or positioning needs and emergency health protocols
- Communicate concerns and potential risks to the appropriate Department Director and/or the Senior Director.
- Other duties as assigned.

Qualifications:
- Registered Nurse (RN) with at least 2 years of nursing experience in Psychiatric or Mental Health facility, preferred
- Registered Nurse (RN) with at least 2 years of nursing experience preferably in Rehabilitation and/or Emergency Nursing and/or working with aging adults and individuals with Developmental Disabilities and/or Autism, preferred
- Current Pennsylvania RN License or LPN License
- Act 33,34 and FBI clearances
- Valid PA Driver’s license
Minimum Compensation:
USD $34.00/Yr.
Maximum Compensation:
USD $34.00/Yr.
