Job Description

We are seeking compassionate and experienced Occupational Therapists (OTs) to join our growing team! We are a leading provider of home health services, dedicated to helping individuals live independently and safely in their own homes.

What You'll Do:

Provide Expert Care: Evaluate and treat patients in their homes, addressing a wide range of needs, including:
Home safety assessments
Medication management
Wheelchair seating and positioning
Pain management
Falls prevention
Management of chronic diseases
Dementia care
Family caregiver training
Collaborate with the Care Team: Work closely with patients, families, and other members of the healthcare team to develop and implement individualized treatment plans.
Educate and Empower: Educate patients and families on self-care strategies, home modifications, and community resources.

What You'll Bring:

Current, unrestricted California Occupational Therapy License.
Current BLS/CPR Certification.
Valid CA Driver's License.
Experience in Home Health: Prior experience in home health care is highly preferred.
Strong Clinical Skills: Excellent assessment, evaluation, and treatment skills.
Exceptional Interpersonal Skills: Strong communication and interpersonal skills with the ability to build rapport with patients and families.
