What You'll Do:
As a Home Health Occupational Therapist, you'll partner with the Case Manager to provide skilled services to individuals in their homes, helping them live safely and productively. Your work may include:
Home safety assessments and modifications
Medication management strategies
Wheelchair seating and positioning assessments
Pain management techniques
Falls prevention programs
Management of chronic diseases and dementia
Family and caregiver training
What You'll Bring:
Current unrestricted CA Occupational Therapist License.
Current BLS/CPR Certification.
Valid CA Driver's License.
Knowledge of local community resources.
Understanding of individual development, human behavior, and the influence of culture on healthcare.
Knowledge of state and federal homecare regulatory guidelines (preferred).
Experience in home healthcare (highly preferred).
Experience with high-acuity patients (a plus).
Experience working with an interdisciplinary team (highly desired).
Case management fundamentals (highly desired).
Self-direction, empathy, and critical thinking skills.
Outstanding interpersonal and leadership skills.
Excellent organizational skills.
Compensation:
The wage range for this position is $120,000 - $130,000 per year. Starting pay is commensurate with relevant experience. We also offer generous bonus incentive plans, comprehensive health benefits, and a 401(k) with matching.
