RN Case Manager Responsibilities:

• Collaborate with physicians, patients, and families to promote quality outcomes
• Manage patient care across multiple healthcare settings to ensure appropriate levels of care
• Prevent inpatient readmissions by assessing and coordinating resources
• Develop individualized case management plans and track patient progress
• Act as an advocate for patients, ensuring their medical and psychosocial needs are met
• Analyze data to identify patients eligible for the Case Management Program
• Assist with utilization management and ensure compliance with health plan requirements

RN Case Manager Requirements:

• Active RN license in the state of Illinois (in good standing)
• Case Management Certification (CCM) preferred
• Current CPR (BLS) certification required
• Associate’s degree in Nursing required; Bachelor’s degree preferred
• 2+ years of RN experience in a hospital or primary care setting
• Previous case management experience preferred
