About the Role:

We are seeking a compassionate and dedicated Care Management Nurse (RN or LPN) to join our team. This role offers a flexible schedule with potential for remote work. As a Care Management Nurse, you will play a crucial role in supporting patients with chronic conditions, ensuring they receive the personalized, coordinated care they need. You'll collaborate with healthcare providers, social workers, and families to create and monitor effective care plans while empowering patients to take charge of their health.

Key Responsibilities:

Develop, implement, and monitor personalized care plans to align with patients' health and wellness goals.

Serve as the primary point of contact for patients and their families, providing updates and addressing concerns with empathy and clarity.

Collaborate with primary care providers, specialists, and other healthcare professionals to ensure comprehensive, coordinated care.

Utilize health coaching techniques to assist patients in managing chronic conditions and making lifestyle changes.

Maintain accurate and timely records of patient interactions, care plans, and health status updates in the electronic health record system.

Support high-risk and chronically ill patients through proactive care management strategies.

Demonstrate excellent communication, organization, and problem-solving skills while working closely with patients and healthcare teams.

Qualifications:

Graduation from an accredited nursing program.

Valid Montana Nursing License (RN or LPN) or a current temporary license.

Previous experience caring for patients with chronic diseases is preferred.

Certifications & Licenses:

Valid Montana Driver's License (as travel may be required).

CPR Certification.
